Leadership Self Assessment Tool 

The Leadership Self-Assessment Tool allows executives, senior managers, and team leaders to assess their effectiveness across 12 key dimensions of leadership, including communication, decision-making, emotional intelligence, equity and inclusion, team development, and organizational alignment.

 

This tool has been used by dozens of organizations to help leaders identify strengths, uncover blind spots, and create actionable development plans.

The Self Care Network administers the tool and compiles your results into a clear, comprehensive report. We send the personalized survey link directly to each participating leader and manage follow-ups to ensure maximum engagement. Once the survey closes, we analyze the data and deliver a user-friendly summary report, ideal for coaching conversations, retreats, or performance planning.

What to Expect

 

Each participating leader will receive a personalized email containing a secure link to the Leadership Self-Assessment. One week after launch, a reminder will automatically be sent to anyone who has not yet completed their survey. You will also receive an update with a list of pending respondents so you can encourage completion within the assessment window.

​

We recommend emphasizing to participants that their honest, thoughtful responses are essential for accurate and meaningful results. The assessment window remains open for two weeks, after which no further responses will be accepted.

​

Within two business days of the survey closing, you’ll receive a detailed PDF report summarizing both individual and aggregate results. The report includes visual dashboards, leadership domain scores, and reflection prompts to guide next steps in growth and development.

All individual responses are confidential and aggregated for reporting purposes.

Pricing

Payment is required prior to the launch of the assessment.

​

Pricing is determined by the number of participating leaders. For instance, if your leadership cohort consists of 40 individuals but only 15 will participate in the assessment, the pricing will be based on those 15.

​

​Up to 15 Leaders: $750.00

16-50 Leaders: $1,200

51-100 Leaders: $1,800

How Results Are Used

 

The Leadership Self-Assessment is designed to be a mirror, not a measurement. It helps leaders and organizations:

  • Reflect on their current leadership culture and capacity

  • Identify development priorities for individuals and teams

  • Build alignment between leadership behavior and organizational values

  • Support executive coaching, retreat planning, or performance development

 

The data collected is confidential and used solely for internal development purposes. Individual responses are not shared publicly or with other participants.

Optional Next Steps

 

Many organizations choose to deepen their learning by adding one or more of the following:

  • Executive Coaching Debrief (1:1 Session): A 60-minute confidential conversation to unpack your results and set leadership goals.

  • Team Debrief Workshop: A 90-minute facilitated discussion with your leadership team to interpret collective findings and co-create an action plan.

  • Comprehensive Leadership Development Plan: A custom follow-up plan outlining actionable next steps to strengthen leadership alignment, communication, and culture.
